Simplex 4090-9802 — 4 inch surface cover with light pipe for IDNet Relay IAM and Monitor ZAM back boxes.
A ZAM or IAM sits in a 4 inch square box and reports its health with a communications LED that flashes roughly once every three seconds. A blank plate hides that LED, so verifying the module means opening a live box. This trim plate closes a surface-mounted box and carries a light pipe, letting the LED be read with the cover in place.
