Simplex 4090-9801 — galvanized steel semi-flush trim plate with LED viewing window for a Zone Adapter Module. Cover plate only; no ZAM is included.
A 4090 series ZAM mounts inside a 4 in. square box and flashes an LED as it is polled, the fastest confirmation a technician has that the address is alive. Covering the box hides that LED, so Simplex offers trim plates with a light pipe to the face of the plate, letting it be read without pulling the cover and disturbing field wiring.

Returns caution: order 4090-9802 instead for a surface mounted box, and add the 4090-9813 adapter plate for a 4 11/16 in. box. Where the LED need not be seen, Simplex allows a plain double gang blank cover supplied by others.
